Adjustment of Status (I-485) Document Checklist

Complete checklist of documents needed for Form I-485 adjustment of status, including identity records, I-94 history, medical exam, affidavit of support, and prior approval notices.

The I-485 checklist helps applicants prepare a green card package inside the United States. Most packets need identity records, lawful entry or status records, birth certificate evidence, a sealed or properly submitted I-693 medical exam when required, financial support documents for family cases, and copies of prior USCIS approvals or receipts.

Core identity records

Passport pages, photos, birth certificate, certified translation, I-94, visa records, and previous immigration notices.

Medical and support evidence

I-693 medical exam planning, vaccination records, I-864 support package, tax records, pay records, and joint sponsor records when needed.

Category-specific proof

Family petition receipts, employment petition receipts, asylum or refugee records, court dispositions, and other records tied to the filing category.

  • USCIS Form I-485, Application to Register Permanent Residence or Adjust Status

    Check the current form edition, category instructions, filing location, and required evidence.

  • USCIS Form I-693, Report of Immigration Medical Examination

    Confirm medical exam requirements, civil surgeon rules, and current vaccination documentation posture.

  • USCIS Form I-864, Affidavit of Support

    Review when affidavit of support evidence is required with an adjustment package.

  • USCIS G-1055 Fee Schedule

    Confirm the filing fee, filing category, and fee exemption or fee waiver posture before filing.

Core Application

  • Completed Form I-485, Application to Register Permanent Residence

    Use the most current version from uscis.gov

  • Two passport-style photographs (2x2 inches)

    Taken within the last 30 days, white background

  • Certified copy of birth certificate with English translation

    Translation must include a certification statement from the translator

  • Copy of valid passport (biographical page and all visa stamps)
  • Copy of all previous immigration documents (prior visas, I-94s)

Medical Examination

  • Form I-693, Report of Medical Examination (sealed envelope from civil surgeon)

    Must be signed by a USCIS-designated civil surgeon, then submitted under current USCIS validity rules

  • Vaccination records showing required immunizations

    Bring records to your civil surgeon appointment to avoid extra costs

Supporting Documents

  • Form I-94, Arrival/Departure Record

    Download your electronic I-94 from i94.cbp.dhs.gov

  • Copy of current visa or approval notice granting status
  • All I-797 Approval Notices for previously filed petitions
  • Form I-130 receipt or approval notice (family-based)
  • Form I-140 receipt or approval notice (employment-based)

Financial Documents

  • Form I-864, Affidavit of Support (signed by petitioner/sponsor)

    Sponsor must show income at 125% of federal poverty guidelines

  • Federal tax returns for the last 3 years (sponsor)
  • W-2 forms for the last 3 years
  • Recent pay stubs (last 3 months)
  • Bank statements showing current balances

Additional Evidence

  • Police clearance certificates from all countries of residence (if applicable)

    Required if you lived in another country for 1+ years after age 16

  • Court dispositions for any arrests or criminal charges

    Disclose all incidents even if charges were dropped or expunged
